It looks like QME's need to begin submitting reports to the DEU with the EAMS cover sheet and separator sheets.

Are there any other filings which QME's may be required to utilize these forms?

Also, would this be the correct filing for an unrepresented case sent to the DEU:

- EAMS Document Cover Sheet (DWC-CA Form 1023.1)
- Employee Disability Questionnaire (DWC-AD Form 100 DEU)
- EAMS Separator Sheet (DWC-CA Form 1023.2)
- Request for Summary Rating Determination (DWC-AD Form 101)
- Findings Summary Form (QME Form 111)
- Proof of Service Form
- Serve report, POS and Summary Form on all parties

Anything you would file the WCAB will need to be done via EAMS. If you need to file a Lien, then yup.. it's done via EAMS.

In the case of a QME report, you'd need to file it this way:

Document Cover sheet (remember to only file as many pages as you filled out)
- Document Separator Sheet for the DWC-AD 101
- DWC-AD 101
- Document Separator Sheet for the DEU 100
- DEU 100
- Document Separator Sheet for your Report
- Medical Report with Form 111
- Document Separator Sheet for POS
- POS

One idea I have been suggesting to my fellow examiners is that we provide the EAMS filing (other than report, Form 111, and POS) to the QME as part of the file. That way the QME's need only complete their report and POS, then assemble the filing and pop it in the mail to the DEU.

It is so great to see that Earth friendly California move "paperless" HAHA. I see the Application for Adjudication is now 5 pages instead of 1. This REALLY saves resources! Hillarious. So we produce these reams of paper which get scanned...and then stread the paper....this seems even more disrespectful and wasteful of resources. But then I was brought up to clean my plate at the dinner table to not be wasteful.

liens as well, if as usual somebody forgets what a QME is and does not pay you.

and not to nit pick, but it should go in this order, not sure if this was your intent...but you need a separator sheet for each document

EAMS Document Cover Sheet (DWC-CA Form 1023.1)
- EAMS Separator Sheet (DWC-CA Form 1023.2)
- Employee Disability Questionnaire (DWC-AD Form 100 DEU)
- EAMS Separator Sheet (DWC-CA Form 1023.2)
- Request for Summary Rating Determination (DWC-AD Form 101)
- EAMS Separator Sheet (DWC-CA Form 1023.2)

- EAMS Separator Sheet (DWC-CA Form 1023.2)
-Narrative Medical Report

- Proof of Service Form

Each document must be separated with a separator sheet so it can be indexed and referenced individually.

As to paperless....why could every ins. co. TPA and Tom, Dick and Harry buy a scanner that was capable of scanning the old forms, but the state had to take three prehistoric steps back and get forms that are as large as eye charts for the visually imparied???

I personally want to be the company who has the contract recycling the shredded paper.

Those "Green Jobs" everyone is so excited about! lol

While I appreciate the effeciencies gained by a "paperless" system, you have to laugh at the shear waste involved. Let us not forget, the postage (and therefore fuel) required to deliver the added pages. With postage going up in April (again...there is a surprise) we are looking at additional waste and expense.

In an effort to decrease paper, I have been requesting medical records in E format (Tif or pdf). I place the request on the QME Notification form (thankfully THIS form is still one page!) I am batting about 20% so far. I got my request into the DWC too late regarding this recommendation. Perhaps in the next round of Forms!

I know, eh? Go peperless to reduce the Clerk's work-load....

Electronic Bulk filing cannot happen quickly enough for me and my company.

Ginger is right. you need to have the separator sheet before each and every document.

For future filings keep in mind that the Cover Sheet and Separator Sheets sole purpose is to tell EAMS what is happening next.

Essentially, the data from the cover sheet is scanned and the barcode tells the system to either create a new file, or to open an existing file. The Separator Sheets say "the following pages are This document issued on this date by this person".

I'm sending out my first Panel QME with the new EAMS forms. I am following the order suggested by gaiassoul1, but have a question. When I'm filling out the separator sheet and for form 111, which box do I check? For form 101 I checked the box "Request for Summary Rating - QME Report." But then form 111 is the Findings Summary Form completed by the doctor, but don't see anything in the Separator Sheet Document Titles that fits that particular form.

If you are referring to which choice on the Separator Sheet; my guess is select "Other Correspondence".

A true paperless system would allow pdf submissions into an FTP. All they had to do is consult, oh, an average 15 year old to figure that out. What a waste of paper.

EAMS is rapidly descending into a Monty Python sketch.

Actually, under "Other" My choices for documents types are: Earnings Information, Job Description, or Photographs.

I think I'll just combine form 101 and 111, behind one seperator sheet. If it gets sent back that I'll deal with it. Just one more glich on the way to paperless!

What's 3 or 4 extra pieces of paper! To get to paperless...we must use MORE paper. Makes perfect sence in an insane world. Even the new Patient Disability Questionnaire is now 2 pages instead of 1. Truely, this is beyond a joke.

I used to work for a company that was trying to go "paperless." The computer was set up to receive bills and reports electronically--so when a new bill came in and was authorized by the C/A to be paid, it generated every single bill and accompanied report to be printed in the history of the claim...

As you could imagine the files that were several years old, reams and reams of paper were generated and wasted. To add insult to injury, the paper wasn't shredded, nor recycled--just thrown in the dumpster along with all of the confidentail information about the I/W.

After over a year of expressing my "concerns," they stopped. All the adjuster had to do was click on a "bill" and a report would be generated/printed.

I'm of the belief (and always have been) that no entity can be completey "paperless." Why? The computers are crashed, not backed up, etc. and if a party proceeds to court, a "paper" file with "real" signatures are needed as evidence.

So what's next, submitting a "disc" copy of a file to the WCAB???

Back in the "day" I remember when all documents, i.e. medical reports, lien objections, liens, etc. had to be served on the WCAB and "two hole punched!" I forget what year that stopped, but in the early '90's the WCAB was so inudated with paperwork, the rules changed and it wasn't until a case was set for hearing that the "file" was submitted,, along with the DOR or Application (forget which).

Ahhh, what I wouldn't do for the "good ole days..."

This is beyond a "joke," along with "Furlough Fridays..." I remember a W/B cartoon with "Wiley Coyote," buying some "water" pills from "ACME," to quinch his thirst in the heartless desert... He needed the water to keep up with the "Roadrunner..." Problem was, he had to add "water" in order for the "water pills" to produce "water..."
