Hello and good afternoon to all. This question has probably been addressed before so I apologize for asking again. Can someone please tell me what information I should have before filing a lien? Our hearing rep has stated that in order for us to file a lien, we should have all the following information; patients name, total charges, date of injury, the employers name and address, the insurance carriers name and address, WCAB # if one has been issued, and the name of the adjuster. I just wanted to know what the minimum amount of information is needed to file because verifying who the adjuster is can be very time consuming. Thanks for any responses.

All of the information would be helpfull. Sometimes claimants have multiple injuries with multiple employers or insuarance companies. Making sure you are filling the lien on the right case is always benficial. It is fustrating appearing on the wrong case. I don't see any reason for needing the adjusters name for filling the lien. But you might need this information to collect the lien.
