Most counties and even some cities provide door to door transportation for disabled or elderly people. They require information as to the to reason the person can't use the regular bus system. Some require a phone or in person interview to confirm the claims as to the disability. Once the paperwork is in, the acceptance or denial is issued in 21 days. If transportation is needed, and the injured worker can't use the bus, there are options.

Attached is the form of LA county, via Access.

The only problem is the counties provide transportation within their county. Travel between counties require a trip to an transfer location and changing to a different transportation program. Setting up travel between counties can be more difficult, but it is not impossible. The cost is very reasonable. LA county costs up to about $10 round trip depending on the distance traveled.
