Lets not get carried away:
The rules on predesignation of a chiro or acupuncturist under LC 4601 are different than for predesignation of a medical physician under LC 4600.
There is no requirement for the chiro or acupuncturist to agree in advance nor is it required that the employer offer group health. Those limitations only apply to MDs, DOs and DPM for predesignation.
Chiro & Acupuncturists may still be predesignated, unless the employer/carrier has contracted with an MPN. If the employee is covered by an MPN, only predesignation under LC 4600 (MD, DO and DPM) allows the employee to escape the MPN. Predesignation of a chiro or acutuncturist is ineffective where there is an MPN.
There is a different form for chiro/acupuncturist predesignation on the DWC website. It explains some of the limitations on going to the predesignated chiro or acupuncturist and does not include the signature requrement as that is not in LC 4601.
