1. Yes, because there is no such thing as a nationwide policy and the insurance companies are licensed state by state by their requirements. So 2. Many but they are probably not licensed for each insurance company in each state, so while companies like AIG, Chubb, etc. write comp in all states, the actual "paper" may each have a different insurance company name, but there are states that have only the State Compensation Fund equivalent 3. Find a smart broker, you need one.
