Dear WCC readers,
Attached you will find a Petition for the LC 4603.2(b) statutory increase and interest, as well as reimbursement of the LC 4903.05 lien filing fee, that I regularly use before the WCAB. This Petition can be used for the following purposes:
1. Getting the increase and interest on unpaid bills;
2. Getting the increase and interest on bills paid properly, but paid late;
3. Getting the interest on bills paid timely, but paid at less than OMFS; and,
4. Getting the LC 4903.05 lien filing fee reimbursed.
The attached Petition is written in MS Word and is a 'form-field' document that is locked. You will note that you can now only access the actual 'form-fields.' In order to unlock the Petition, and access all parts of the Petition so as to customize if for anybody's use, please follow the directions below:
You must be able to view the 'forms' toolbar in MS Word. First, as a suggestion for organization, I would create a new folder in "my documents' called "BRIEFS." Then, download this Petition and save it in the new folder called "BRIEFS." Next, start MS Word and open the Petition.
In order to view the 'forms' toolbar while in MS Word, open the Petition then click on "view" and slide your curser to "toolbars" ----- then slide your curser to "forms" and click once. You will now see the "forms" toolbar. Please note that there is an icon that looks like a padlock. Click once on the padlock and the Petition will be unlocked. You can now access all portions of the Petition and change, or delete, any language you want.
Once you have customized this Petition, be sure to click on the padlock to lock the Petition again. Now, you will only have access to the 'form fields' again. Be advised that if you start to enter data into the 'form fields' and decide to unlock the Petition to change some language, if you lock it with any data in the 'form fields,' all data previously entered in the 'form-fields' will be deleted, and you will have to start entering data from the beginning.
If I have begun to enter data, and decide to delete inapplicable sections of language, I unlock the Petition, make my changes, and continue working on the document in an unlocked state. When I am finished, I save the document with a different name, indicating the name of the IW. (e.g. The default Petition is called "PETITION-4603.2(b).doc." When I save it, I will rename it "PETITION-4603.2(b)-SMITH.doc.")
By saving the modified Petition with a different name, the default "PETITION-4603.2(b)" remains locked and intact, and I now have saved an additional customized copy for use before the WCAB, for that particular IW's case, and print it out.
