Hi WCC Forum Members:
Below you will find a copy of a DWC Audit Unit complaint letter, from an IW, for UR violations.
This letter is written in MS Word, and it is a 'form field' document that is locked, but can be unlocked by anybody for customization. To unlock this letter, while in MS Word, click on the "View" button, scroll down to "Toolbars" then select "Forms." At this point you should see a new set of icons, one of which looks like a "padlock." Click on the padlock and the letter should then unlock for customization. To be safe, I will also attach an "unlocked" version that can be locked after it has been saved to your computer.
